The form below must be completed by the distributor that has purchased the products from DAB PUMPS UK (includes Southern Ireland claims). A purchase order is required when submitting the form, to cover the site visit cost should the issue not be covered under warranty.

We require as much information as possible to correctly handle your request, specifically the data plate label on the product/s and original invoice.
